Good Faith Estimate Notice

Your Right to a Good Faith Estimate

Under the No Surprises Act, healthcare providers, including Essential Health Chiropractic, are required to provide patients who do not have insurance or who are not using insurance with an estimate of the expected charges for medical services. This estimate is called a "Good Faith Estimate."

  • When you schedule an appointment for a service.

  • If you request one before scheduling a service.

  • No later than 1 business day after scheduling an appointment if the service is scheduled within 3 business days.

  • No later than 3 business days after scheduling an appointment if the service is scheduled more than 3 business days out.

What Should You Do If You Receive a Bill That Is Higher Than Your Good Faith Estimate?

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill. To dispute the charges, you may:

  2. File a Dispute: If the issue cannot be resolved directly with our office, you may initiate a dispute resolution process through the U.S. Department of Health and Human Services (HHS). You must file your dispute within 120 calendar days (about 4 months) of receiving the bill.
